If you have cataracts and are considering cataract surgery, you might have heard about the benefits of diffractive Extended Depth of Focus (EDOF) IOL’s such as Synergy or Vivity. These lenses are effective in improving vision and eliminating refractive errors, but they come with a great expense due to the echelettes on their surface that are responsible for increased depth of focus but also cause halos, glare, and cobwebs.
Fortunately, there are now non-diffractive EDOF IOL’s such as Rainer One EMV and Tecnis Eyhance IOL’s that offer a more affordable option with similar benefits. These lenses have no diffractive echelettes on their surface but have highly positive spherical aberration that extends the depth of focus up to 1.5 diopters (see illustrations below).
One of the advantages of non-diffractive EDOF IOL’s is that they can be used in a blended mono vision arrangement, with one eye better focused at distance while the fellow non-dominant eye is focused at near. This is particularly useful for patients who want to regain multifocality without the need for glasses.
Because non-diffractive EDOF IOL’s have extended depth of focus, there is a lot of overlap in the middle distance where stereoscopic vision is at a premium. Additionally, these lenses are more forgiving to minute decentrations or imperfections on the cornea, making them a more reliable option.
Best of all, patients who choose non-diffractive EDOF IOL’s rarely need glasses of any kind to read, drive, use a computer, or watch TV. There are no halos or glare at night, which is a common concern with diffractive IOL’s.
